John, try a baking soda wash that is pretty heavy on the soda....and let it stand in it to absorb the smell. After that wipe it out with pure vinilla extract (use fairly liberal dose).
I haven't tried it with a plastic water cooler, but on our water cooler at home that has a stainless steel holder, I fill it with hot water and pour in enough vinegar to get a strong vinegar smell and let it sit overnight and then drain it--this gets the odors and mineral accumulation out and works fine.
Very hot water with vinegar has been the cure for a couple coolers that I have. The hot water seems to soften the plastic just enough to pull the fumes out. As Martin said, let it sit for a good while (at least a couple of hours). I then run a couple tanks of pure hot water in it and it's usually fine.
